Best Chance Network Program (Women’s Cancer Screening)
The mission of the Best Chance Network (BCN) program is to reduce mortality from breast and cervical cancer by funding comprehensive screening services for women who meet age, income and insurance eligibility criteria, thereby finding cancers at earlier and more curable stages. Our partnership with the American Cancer Society’s BCN Program, provides participants a variety of screening and diagnostic services to women ages 30 – 64 including clinical breast exams, mammograms, Pap smears and some follow-up tests.
Case Management Services
Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
Language Interpretation
To provide the best care, health care providers must be able to communicate effectively across languages and cultures. At New Horizon Family Health Services our Certified Medical Interpreters work to eliminate language access as a barrier to high quality health care. We have the ability to interpret up to 150 languages, including sign language for the deaf and hearing impaired.
